Case Management - South West Sydney

This service at our South West Sydney branch helps parents deal with issues resulting from a child's developmental delay or learning disability.

The service is available to families who have children who:

are aged 0-18 years;
have a mild to moderate developmental disability (such as emotional, cognitive, physical, behavioural, or sensory difficulties) or a moderate to severe learning disability;
if under five years, are suspected of having a mild to moderate developmental disability or are at risk of not achieving developmental milestones without intervention;
live in the Bankstown, Liverpool or Fairfield local government areas; and
are not already accessing case management support through the NSW Department of Ageing, Disability and Homecare or other case management services.

Our family counsellors offer:

short to medium term case management and coordination of support services;
information, advocacy and referral to relevant specialist and mainstream services;
detailed assessment of the individual, his/her family and carer needs;
Individual Family Service Plans (IFSP) addressing identified needs and priorities;
family intervention and supportive counselling for families experiencing difficulties communicating, managing behavioural problems or dealing with recurring issues; and
group work for children and their families.

Interpreting assistance is available for families with languages other than English.
